Purchase order

To order by mail, print and fill out the purchase order form and return it to 377, La grise Lake Road, Sainte-Agathe-des-Monts, Quebec, Canada J8C 2Z8, with a postal or bank money order (cheques are not accepted) payable to Perfect Life Institute in Canadian currency. There are fees to pay to obtain a money order. You can also fill out the form directly on the computer and print it.

Canadian taxes are not included for Canada (6%) and Quebec (6% + 7,5%) only. They must be added to the subtotal less the rebate (if applicable) and the handling and shipping. For all other countries, you don't have to add any taxes.

When duties or local taxes are applicable, they have to be paid by the customer at reception of order.

Handling and shipping

The handling and shipping costs vary according to the number of items ordered. They are calculated automatically. The shipping will be as fast and secure as possible upon reception of full payment.

For 1 or 2 items:
- it is $ 10 CAD for Quebec
- $ 12 CAD for any other Canadian province
- $ 14 CAD for the United States
- and $ 28 CAD for any other country
For 3 to 5 items:
- it is $ 12 CAD for Quebec
- $ 16 CAD for any other Canadian province
- $ 20 CAD for the United States
- and $ 58 CAD for any other country
For 6 and more items:
- it is $ 14 CAD for Quebec
- $ 20 CAD for any other Canadian province
- $ 26 CAD for the United States
- and $ 88 CAD for any other country

Volume rebate

A rebate is offered to all purchases of $ 500 and over (handling and shipping rates not applicable). This rebate is not cumulative : it is offered on one purchase only.

For a purchase of $ 0 to $ 499 CAD, you pay the current price.
For a purchase of $ 500 to $ 999 CAD, 5% rebate.
For a purchase of $ 1000 to $ 4999 CAD, 10% rebate.
For a purchase of $ 5000 and over CAD, 15% rebate.

In each appropriate box, please indicate the quantity (Qty) of the selected item which you will multiply by the corresponding Canadian price CAD to obtain the amount for each item. Add these amounts to obtain the subtotal. Deduct, if applicable, the quantity rebate. Add then the handling and shipping rate to obtain the total amount.
